Any information entered in the application or sent to PLIA is subject to public disclosure. Do not submit any information or supporting documentation that contains sensitive information, such as a Social Security number, taxpayer identification number, driver license number, or date of birth. We strongly advise that you blackout or remove personal identifiers.

For applicants who are unable to submit the online application or require translations, alternative application forms are available on request. To request an alternative format or translation of the application, please contact PLIA by email at pliamail@plia.wa.gov or call 1-800-822-3905.

Once an application has been submitted, PLIA will review the application. If additional information or documentation is necessary, PLIA staff will contact the applicant.

If PLIA determines your UST is not eligible for the FA Program, we will provide an explanation and we may list the requirements to become eligible. If you meet the eligibility requirements, you can re-apply for entry into the FA Program.

Once PLIA determines that your UST is eligible for the program, we will send you an Enrollment Agreement. The Enrollment Agreement outlines the terms and conditions of enrollment, the coverage that will be provided, and any requirements specific to the UST facility. The enrolling owner or operator will sign the agreement and submit it to PLIA. Please note: Financial Assurance Program coverage does not begin until PLIA signs the agreement, receives payment for the enrollment fee (or an installation payment if you are on a payment plan), and sends a Certificate of Assurance to the applicant.
